Lisa Graham, MSN, RN-BC, is the Senior Director Integrated Care Management and Quality at The Ohio State University Wexner Medical Center. Lisa has supported OSU for more than 18 years and is a thought leader on effective case management techniques. In her role, she has led various innovative solutions in Case Management to improve staff efficiencies and support optimal patient outcomes.

Marcia Colone has dedicated over 25 years of her career to the delivery of care coordination services at large academic health systems. Since 2017, Marcia has been the Vice President of Vanderbilt Medical Center’s Transition Management and Post Acute Care Department which has a staff of over 165; comprised of administrative directors, managers, case managers, social workers and discharge planners.

Paul has spent most of his 25+ years as a nurse in leadership positions including being a Chief Nurse Executive and Director of Patient Care. More recently he has spent the last 17 years in case management in leadership positions as a Director, Senior Director, Executive Director and Assistant Vice President. Paul has also authored a book entitled Prevent Denials and Win Appeals: The Hospital Case Manager’s Guide to Revenue Integrity as well as having published articles and doing lectures across the country for the ACMA and other entities. Paul has also authored a core curriculum for case management that has been used by several community colleges in the greater Los Angeles area.
